Smart Life Forum, Inc. (SLF) is a California nonprofit corporation organized under state law for educational and scientific purposes as a public benefit corporation. Our primary mission is to provide credible health education to the public with an emphasis on optimal wellness, anti-aging medicine, and longevity. 

SLF is approved for tax-exempt status by the IRS under section 501(c)(3) of the Internal Revenue Code and by the State of California under section 23701(d) of the California Revenue and Taxation Code. As such, donations to Smart Life Forum, Inc. are tax deductible to the extent allowed by federal and California state law. 

Deductible portion of membership dues

SLF membership is deductible to the extent allowed by law. In figuring your deduction, you must subtract the value of the SLF Newsletter ($15) if you receive it in the form of a printed copy by postal mail. In that case, if you paid $60 for your membership, the deductible portion would be $45. However, if you receive the SLF Newsletter by email, which saves us the cost of printing and postage, then the deductible portion would be the full $60.

Contributions - cash & noncash

SLF, a Public Benefit Corporation under California state law, is treated as a "public charity" under its 501(c)(3) federal tax exemption from the IRS. As such, both cash and noncash contributions to SLF are deductible to the extent allowed by law.

For charitable contributions of $250 or more, current IRS regulations require that the donor get a written statement from the charity listing any cash contribution or describing any donated property. There is no prescribed format for the written statement. Letters, postcards, and electronic (email) or computer-generated forms are acceptable.

The donor is responsible for the valuation of any donated property. However, the charity’s written statement must provide sufficient description to support the donor's contribution.

How to make a contribution

Cash donations can be made by mail or in person at any of our monthly meetings. Please make your check payable to "Smart Life Forum, Inc." and give it to our Chief Financial Officer, Laurel Corcoran, at the Registration Table or mail it to:

For contributions of $250 or more, Laurel will automatically provide you a receipt, but she will be happy to provide a receipt for any amount, if requested.

Noncash donations of electronic equipment or other items can be made by bringing the items to any of our monthly meetings. Our Chief Financial Officer, Laurel Corcoran, will be happy to provide you written substantiation of the item(s) you donate. (Valuation to be specified by you.) If you have a non-cash item that you would like to donate, please contact Laurel (Laurel Corcoran) ahead of time to find out if it is something that SLF needs and could use.
